Help with your long-term condition review

Living with one or more long-term conditions can be tough at times. That’s why York Medical Group offers a yearly health review to check your health and make sure your condition is well managed.

We’ll write to you when it’s time to:

  • Send your blood pressure readings.
  • Do a blood or urine test.
  • Complete a virtual assessment.

There’s no need to book an appointment for when your check-up is due. Our team reviews your answers and contacts you if needed, so you save time and trouble coming down to the surgery. You’ll get an up-to-date, personalised care plan to keep you on top of your conditions too.

And if you prefer to do things the traditional way or you don’t have the internet, just give us a call or pop in and see us. Our reception team can help you.

How long-term conditions reviews work

We’ll invite you to have your long-term condition review once a year. If you have more than one long-term condition, we’ll review them at the same time.

You’ll need to:

Check your blood pressure

Do this at home or pop into the surgery. We have machines in all our reception areas.

Get your tests done at a time to suit you

If you need to do a blood or urine test, we’ll tell you in your letter.

Choose to have your test done at a walk-in service (no appointment needed) or give us a call on 01904 439100 if you can’t make weekdays and we can book you an appointment locally.

Complete your virtual assessment

Visit www.virtualhealthassistant.co.uk and use the access code we send you to fill this in at home.

Depending on your condition, we’ll ask you about:

  • Your height and weight
  • Alcohol and nicotine use
  • How well you take your medicines and if they’re still working well for you
  • What happens when you’re feeling worse
  • Your physical activity

Our team review your answers and contact you to discuss if they need to. There’s no need to book an appointment.

You’ll receive an up-to-date personalised care plan so that you and your family know how to manage your condition and what to do if your symptoms get worse.

Keeping on top of your health conditions

Long-term conditions such as asthma, diabetes, chronic obstructive pulmonary disease (COPD) and arthritis are all conditions we don’t currently have cures for but can manage with medicines and other treatments.

As your GP practice, we play a key part in helping you manage your conditions. Whether it’s reducing your symptoms, changing your medication or introducing new treatments that become available, our team are here to help you live your best life and stay as healthy and well as you can be.
