Healthcare Assistants & Assistant Practitioners

Our team of Healthcare Assistants and Assistant Practitioners are trained to care certificate standards. Clinical skills include, blood pressure monitoring, health promotion, phlebotomy, ear care, recording electrocardiograms, urinalysis, acting as a chaperone, wound care, management of long-term conditions, administration of injections, assisting with minor surgical procedures and contraceptive clinics. Assistant Practitioners are also trained to provide ankle brachial pressure index measurements and management of asthma.


Practice Nurses & Nurse Prescribers

Practice Nurses are qualified registered Nurses who work autonomously. They provide contraceptive, sexual health and cervical screening services. They are responsible for the management of long-term health conditions including respiratory, diabetes, hypertension and cardiovascular disease. They deliver immunisation programmes, wound management and are accomplished in treating minor ailments.


Allied Health Care Professionals

Allied Health Professions are a diverse group of clinicians who deliver high-quality care to patients across a wide range of care pathways.

Our team includes Paramedics, Physiotherapists and Nurses. They offer both same day care and routine appointments in their specialist field. The team can help with assessment and diagnosis of acute and chronic conditions. Prescribe and review treatments for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice.


First Contact Mental Health Care Practitioners

Our First Contact Mental Healthcare Practitioners are registered Mental Health Nurses. They assist patients and offer short term interventions which may include anxiety management, support with low mood, advise on possible medications and work with a range of common mental health issues.


Complex Care & Visiting Team

Our complex care and visiting team are made up of a multi-disciplinary team including, Healthcare assistants, Assistant Practitioners, Practice Nurses, Paramedics, Emergency care Practitioners and Doctors. This team provide services for our housebound and vulnerable patients to support them with both acute and chronic health issues and palliative care.

Chief Executive Officer

The Chief Executive Officer is involved in leading and managing all aspects of the practice, ensuring the appropriate governance and systems are in place to provide the highest quality patient care. 

The Chief Executive Officer supports the GPs and other medical professionals with delivering patient services and also helps to develop all aspects to enhance patient care.


Polly Smith

Head of Clinical

Head of Clinical manages the clinical team at YMG across all sites, she ensures CQC and Infection Control standards are met and the QOF targets are reached.

She manages the clinical data and templates, LES/DES adherence, and oversees the Administration Team with patient recalls.

She resolves any clinical issues and coordinates staff holidays, clinical staff training, and works alongside the Chief Executive Officer. 


Tess Johnston

Head of Operations

Head of Operations, manages the operational side of YMG. Tess ensures that systems are in place to enable all YMG sites to operate smoothly and efficiently.

Tess works closely with Zulf Ali and Polly Smith to deliver a high-quality customer service to all our patients.

The Administration Team process all new registrations, deductions, and recalls for all of our sites.

They also process all patient vaccinations and results so they are ready for the Doctors; the team also co-ordinate clinical schedules and maintain the health and safety of the building.

The administration team also deals with all Private work. This includes medical insurance reports, holiday insurance claim forms, DVLA reports, and D.W.P forms. All private work carries a fee.

Clinical Coders

The Clinical Coders action and process all hospital correspondence; they code patient diagnosis and investigations, thus keeping the patients records up to date.


Patient Care Co-Ordinator’s

Patient care co-ordinators provide an important link for patients with the practice and are your initial contact point for general enquiries.

They can provide basic information on services and results and direct you to the right person depending on your health issue or query; patient care co-ordinators make most of the patient appointments with the GPs and nurses.

They also perform other important tasks such as issuing repeat prescriptions and dealing with prescription enquiries, dealing with financial claims, dealing with patient records and carrying out searches and practice audits.

At York Medical group we have a dedicated Pharmacy team consisting of Pharmacists, Pharmacy Technicians and Prescription clerks. The team undertake medication reviews including supporting patients to get the best outcome from their medicines and co-ordinate medicines for patients following discharge from other healthcare settings. They also conduct audits & improve prescribing practice through educational support for all prescribers within the practice and to help improve prescribing and patient safety. They also provide a contact for patients and colleagues with medication queries. 

We are York Medical Groups complex care co-ordinators; we are here to help you. We liaise closely with your GP to provide continuity of care. Our aim is to ensure you feel supported and have access to the services that are right for you. We develop tailored personalised care plans for patients identified with significant long term and complex health issues and social care needs.

To give you an idea of how we can help:

  • We work closely with the home visiting team and GPs to ensure continuity of care and arrange home visits.
  • Telephone patients after discharge from hospital to ensure they have the support they require.
  • Refer to District Nurses or Palliative care support.
  • Support Care home, residents, and their families.
  • End of life/Complex Needs.
  • Arrange dementia, learning disabilities and mental health reviews for patients.
  • Liaise with Social services to co-ordinate care.
  • Visit our house bound patients to deliver vaccines and long-term condition reviews.
  • Refer carers to specialist support to help them look after their loved ones.
  • Arrange for Social Prescribers to offer support and sign posting to other services.
